Where do I send Form 1583 and my IDs?

Please send the notarized form and copies of your two IDs to us by using one of the methods below:

Please make sure to have the Form 1583 notarized.  Alternatively, the notary requirement can be waived if you submit a selfie photo of yourself holding your photo ID.  In that case, you do not need to notarize the form.  Just send the signed Form 1583, the two IDs, and the selfie.
